Medical Form and/or Disability Application - Nurse Practitioner

Our Nurse Practitioners (NP) complete health forms for school and work, as well as disability paperwork. As part of this process, we may need to see you for an in-person assessment if required, depending on the form being completed. The first step in having a disability application completed, is to book an initial “discovery” appointment with our NP. This appointment with provide a preliminary review of your health and function and medical history, to determine if completing a disability application is the right decision for you. For disability applications to be completed, we require medical documents to review, such as your medical chart (this can be accomplished by faxing or emailing via our encrypted email service). This may require additional visits, and the fee to complete disability paperwork may differ based on the amount of information to review and complexity of the application.
